Background:Global health emphasizes population health, equality, and positive thinking. Enhancing people’s literacy in life, death, and medical autonomy involves the following: 

  1. Promoting the public’s ability to access, process, and understand essential health information and services, empowering them to make informed health decisions. 
  1. Building capacity to continuously learn to face life and death issues, at any stage of life. 
  1. Acquiring, understanding, and applying knowledge and skills related to end-of-life and palliative care. 

MacKay Memorial Hospital actively connects the community care network and community care bases, Community psychiatric rehabilitation institutions, long-term care institutions. We develop and establish new cooperation models for community medical outreach services- Promote and protect the medical autonomy and hospice rights of people with cancer in the community and long-term care systems. 

Methods/intervention : 

We offer both "Institutional ACP" and "Home-based ACP" services. In the first phase, we conduct propaganda to enhance understanding of the Patient Right to Autonomy Act, Advance Care Planning (ACP), and palliative care for individuals with cancer patients, their families, and institutional caregivers. We provide education courses on medical autonomy and palliative care for community institution personnel. The second phase involves setting up "Advance Care Planning (ACP)" outpatient services to help individuals sign Advance Decisions (AD) and express their willingness to receive palliative care. 

Results : In 2023, we held 60 medical autonomy awareness lectures, with a total of 158 cancer patients signed Advance Decisions (AD). From January to September 2024, 175 cancer patients have completed signing Advance Decisions (AD). The 333 cancer patients were mostly female, and most of them were diagnosed with breast cancer and lung cancer.
